Automatic numbering is inaccurate with similar footnotes present

Description
If a page has two footnotes with same content, they will have the same reference number, which is OK. But next reference will have innacurate number (+1).

This bug should be fixed in the latest release of the plugin, 1.2.2. I didn't end up using your patch but I do appreciate the bug report.
